At Pure, Perla, and Platinum, you can book a 40-minute session for $150, which covers your $50 room fee and $100 for the attendant. You get an exotic massage for that. Studio9 is in the same chain and the pricing scheme is a little different. The 45-minute option may be best there, at $170, but you can get 30 minutes for $130.

What is covered by "exotic massage" does vary from girl to girl, but at the Premier chain, it tends to be very friendly service. But if you want to put it in somewhere, you should expect to pay extra for that. Not every attendant make it an option, either, but you can find information about many of them on this board. Typically you should tip $50 for a BJ or $100 if you get FS (which often includes a BJ to get you started). So you're looking at $250 for a 40-minute session.

I have suggested in other threads that people who are looking for FS might be better off with an escort incall. Many of them are in the $250 range for a 60-minute session.
